Sarah Jones was an assistant camera operator working on the film, Midnight Rider, when she tragically lost her life due to unsafe work conditions. Currently the producers and first AD are being sued for involuntary manslaughter and criminal trespassing. Click here and here for more information about the case.
Something positive thats come of this is a new app (pictured below) that allows anonymous reporting of unsafe conditions onset to the proper authorities. The app is available for both IOS and Android phones.
